Has anyone ever came across a case of click fraud that was taking place via email spamming, where several million of people receive a spam email consisting of an obfuscated link with multiple redirects, eventually resolving in a paid clicks having zero value to the advertiser when the email recipient clicks on the link. Or even worse, the click being triggered just simply by opening the email (e.g. the link being a beacon hosted on an ad network server).

Yes, I've seen that in PPC campaigns, but I'm wondering if this and other schemes could be email-based rather than browser-based.

It would certainly be possible. As soon as the email opens, a pixel could fire and ping a server and set off the triggered events.
